Step Forward Wellness is in-network with Priority Health Insurance. Using Priority Health Insurance with our Grand Rapids physical therapy clinic is easy. Simply contact us with the link below and share the information on your Priority Health Insurance card and we will verify your benefits and then get you scheduled. Once you do that, your therapist will make sure to use our secure service to submit your sessions to Priority Health Insurance. 

Depending on your insurance plan, you may owe a copay, co-insurance, or a deductible fee at the time of your physical therapy session—this will be collected by your therapist at the end of session. You may use your HSA card at this time if you have one. Other times, Priority Health insurance may pay for your physical therapy session completely depending on your insurance plan and you may owe nothing for the service. To understand more about your insurance plan, please contact Priority Health insurance and ask them what they cover in terms of “Outpatient Physical Therapy.”
