For Physical Therapy

Step Forward Wellness is in-network with Blue Care Network Health Insurance. Using Blue Care Network Health Insurance with our Grand Rapids physical therapy clinic is easy. Simply contact us with the link below and share the information on your Blue Care Network Health Insurance card and we will verify your benefits and then get you scheduled. Once you do that, your therapist will make sure to use our secure service to submit your sessions to Blue Care Network Health Insurance. 

Depending on your insurance plan, you may owe a copay, co-insurance, or a deductible fee at the time of your physical therapy session—this will be collected by your therapist at the end of session. You may use your HSA card at this time if you have one. Other times, Blue Care Network insurance may pay for your physical therapy session completely depending on your insurance plan and you may owe nothing for the service. To understand more about your insurance plan, please contact Blue Care Network insurance and ask them what they cover in terms of “Outpatient Physical Therapy.”

Please note that Blue Care Network insurance is actually a separate healthcare plan that is affiliated with Blue Cross Blue Shield, but is not the same as Blue Cross Blue Shield. Be sure to call your Blue Care Network insurance representative to ask questions about using your insurance for physical therapy. If your Blue Care Network insurance representative has questions, please have them reference your therapist’s name and let them know that the official group name is “Step Forward Wellness LLC.”
